Social Work

We aim to support, and help patients, their families and informal carers to access the most appropriate services and care available to them. We want everyone who uses our services to lead their lives as fully and as independently as possible. We work with organisations and other agencies on behalf of anyone with specialist palliative care needs.

We understand, have a great deal of experience and work closely with you to assess how we can help with:

  • Social needs
  • Personal care
  • Psychological needs
  • Financial problems / issues
  • Employment concerns
  • Family and carer needs
  • Providing / managing care packages
  • Bereavement worries / needs
  • Funeral arrangements
  • Life and end of life decisions
  • Risk and vulnerability assessments

By liaising with other organisations on your behalf, we can offer you help, support and sign-posting to ensure you obtain the most appropriate package from a range of services and government departments.

All patients, their families and carers who use the Priscilla Bacon Centre can easily get hold of the on-site Social Work team. Additionally, anyone who has specialist palliative care needs in Norfolk is welcome to get in touch to see how we can help (see the Contact Us page)

The Social Work Team also offer education and professional consultancy to other health and social care professionals upon request.
